Caregiving Support From Leading Home Health Care Companions

CareTribe and Home Helpers of Monmouth County have launched a partnership to provide free platform access and local resources to help families make decisions earlier.

Freehold, NJ – July 12, 2021 – CareTribe and Home Helpers of Monmouth County are partnering to provide support for unpaid family caregivers, who, in turn, are providing support to elderly, injured, or disabled individuals. This partnership is designed to create better health outcomes and quality of life not only for those needing care but also for the family caregivers through personalized solutions.

Co-founder of CareTribe, Dr. Madhavi Vemireddy, and owner of Home Helpers of Monmouth County, Howard Stein, partnered together to enable a family-centered care model to support the family’s overall well-being by incorporating the best practices of population health programs from their experience working together at ActiveHealth Management, a CVS Health company. Residents of Monmouth County can participate in this new model of care to make it easier to evaluate options and plan proactively for caregiving needs, rather than in reaction to the declining mental, social, or physical health of our loved ones.

Families that engage will find resources that also make time to focus on supporting the family caregiver’s own well-being by gaining access to digital LifePaths tailored to specific caregiving situations and a dedicated Care Navigator who are master-level social workers. Home Helpers assists aging or differently-abled individuals by organizing specialized, personal, or companion care, nutritional counseling, and more with in-home health care to ultimately support care at home and create peace of mind.

Exploring options to decrease the stress, strain, and challenges associated with caregiving, CareTribe helps build resilience in unpaid family caregivers by guiding them to be better prepared for the overall experience. CareTribe serves as a digital platform that combines advanced analytics, personalized pathways, and access to dedicated Care Navigators.
